A recent report from the Institute of Medicine is quite sobering. The report asserts that diagnostic error is underreported, is growing at an alarming rate, and is not being addressed:

  The average hospital pays out hundreds of thousands of dollars per year in malpractice awards and settlements - the majority are associated with diagnostic error

  The largest malpractice awards are associated with diagnostic error

  A quarter-million patients die in US hospitals each year due to preventable medical errors - 10% of those are attributed to diagnostic error

  Up to 17% of adverse events in hospitals can be traced to diagnostic error

  Most adults in the US will be impacted at least once in their lifetime by diagnostic error, oftentimes with devastating consequences

  5% of U.S. adults who seek outpatient care each year experience a diagnostic error
